Provider Agreement and Parent Declaration
Provider Agreement
A form based on the model agreement from the Department for Education (DfE) which provides a framework that outlines the mutual commitments on delivering the early education free entitlement. It is re-issued annually.
The form includes the following among other things:
- Lewisham Early Entitlement teams responsibilities to providers
- Providers’ responsibilities in delivering free entitlement
- Details of the different funding types and how to access them. For example, EYPP, DAF & Inclusion fund
- Eligibility criteria
- Quality
Parent Declaration
The Parental Declaration is for parents/carers to complete and sign.
This should be used to obtain information for funding entitlement claims.
The Parental Declaration and the child’s birth certificate should be retained for auditing purposes in line with GDPR.
If a child takes up the entitlement from multiple providers, parents/carers are required to complete this for each provider in order to reduce funding duplicates.
