Early Entitlement - Updated Conditions of Registration and Eligible Provider Agreement

The Conditions of Registration and Eligible Provider Agreement, September 2024 has now been updated.

This agreement replaces the 2023 version.

You are required to sign and return the provider declaration (appendix 4) form at the back of the document in order to deliver early entitlement funded places.  Please keep a copy for your records.

The signed declaration form must be returned before Friday 15th November, if we do not receive your completed declaration by this date it will delay your final payment.
