Some key changes:

The referral form has the same format as our old referral form. You will see that it has additional questions to help us better understand the child’s needs so that we can map children to the most appropriate intervention for them.

On the first page, you are required to tick if the referral is for Feeding and/or Communication. It is really important that the relevant boxes are ticked so the referral is processed by the correct team.

  • If the referral is for communication only, you are required to complete ALL parts of forms A and B.
  • If the referral is for feeding only, you are required to complete ALL parts of forms A and C.
  • If the referral is for feeding and communication, forms A, B and C must all be completed.

We ask that you answer all questions on the referral form so that we can understand the child’s needs and what is wanted from the referral.
