Updated Parental Declaration – Please Use the New Form

The Parental Declaration form has now been updated and must be used for all new parents/carers going forward.

A key change has been introduced regarding address verification:

• Parents are now required to provide proof of their address when completing the declaration.
• This evidence does not need to be stored in the child’s file.
• A staff member must simply see and verify the document before the form is submitted.

Please make sure all staff involved in completing or processing declarations are aware of this update.
